Completed forms may be submitted by fax to 416 477-5499 “Attention: Ontario Resident Mare Program" or by email to ontariomare@ontarioracing.com, provided that arrangements have been made for the remittance of all required fee payments. Ontario Racing accepts no responsibility for the receipt of faxed application forms by the required deadline. It is recommended that you retain a fax transmission receipt to confirm the date/time of the fax transmission.
